Study of reconstructive surgery for partial resection of the hypopharynx and larynx-Discussion concerning application of a jejunum patch

Summary We performed partial resection of the hypopharynx and larynx simultaneously with reconstruction using free tissue implants in 23 cases during the period from 2004 to the end of 2011. The reconstruction materials used included 18 jejunum patches, 4 forearm skin flaps and 1 anterolateral thigh skin flap. Reconstruction was performed with the jejunum in 16 cases for which the arytenoid cartilage of the larynx was preserved and reconstruction with skin flaps was performed in 5 of 7 cases from which the arytenoid cartilage was removed. The implanted tissue was engrafted in all cases, but a postoperative fistula was formed in 1 case of the jejunum and 4 cases of skin flaps. Among 21 cases excluding 2 cases of death due to metastasis in the lungs, ingestion was possible in 16 cases, but tubal feeding remained due to accidental swallowing in 5 cases for which radiation chemotherapy was added. In the partial removal of the hypopharynx and larynx, the quality of reconstruction of the aryepiglottic fold was closely related to the prevention of postoperative accidental swallowing. For reconstruction of the aryepiglottic fold, it was reported that reconstruction using forearm or anterolateral thigh flaps is successful. However, we think that satisfactory results can be obtained by reconstruction using a jejunum patch in cases where the arytenoid cartilage was preserved.
