Details of operative procedures of esophageal reconstruction using long pedicled jejunal grafts after resection for cancer of the thoracic esophagus are presented. In addition, functional characteristics of three kinds of esophageal substitutes, namely, gastric tubes, pedicled jejunal segments and pedicled colonic segments are analyzed. Typical intraluminal pressures and autonomic contraction of the three kinds of organs were found to be diverse. In the patients examined, the frequency of complaints of post-operative symptoms such as bloating, diarrhea and stenotic sensation were observed to be various in the early periods after surgery but became almost equal 2 years post-operatively, even though the reconstructive routes and/or organs for substitutes were different. The 5-year survival rate of the cases which underwent reconstruction with jejunal segments with curative resection was 32.1%.
