Localization of lymphatic leakage site in chylothorax by thoracic duct scintigraphy by orally administered 123I BMIPP using SPECT/CT

A 58-year-old man with chylothorax after graft replacement of the aortic arch underwent thoracic duct scintigraphy by orally administered 123I-labeled 15-(4-iodophenyl)-3(R,S)-methylpentadecanoic acid with single-photon emission computed tomography combined with computed tomography (SPECT/CT). SPECT/CT fusion image clearly showed the increased uptake near the vascular anastomotic site of the aortic arch. Therefore, a chylothorax due to a traumatic rupture of a thoracic duct was suspected. This report indicates the clinical usefulness of thoracic duct scintigraphy by orally administered 123I-labeled 15-(4-iodophenyl)-3(R,S)-methylpentadecanoic acid using SPECT/CT for the localization of the lymphatic leakage site in chylothorax.
