Correlation between IgM type anti-HBc and clinical state of HBV infection

Correlation between clinical state of HBV infection and IgM type anti-HBc was examined by IMx CORE-M (Abbott, USA), which is sensitive and semi-quantitative. The mean values of cut-off indexes of IMx CORE-M were 2.908 in acute hepatitis (AH) B, 0.921 in acute exacerbation of chronic hepatitis (CH) B, 0.184 in HBeAg-positive asymptomatic carriers, 0.712 in HBeAg-positive CH, 0.661 in HBeAb-positive CH and 0.136 in HBeAb-positive asymptomatic carriers. The cut-off index of AH-B was significantly higher than that of acute exacerbation of CH-B (p< 0.0001). Inhibition percentage of 200-fold diluted serum below 95 percent by IMx CORE (IgG type anti-HBc) was suggestive for AH-B, too. In HBV carrier, most cases with cut-of index above 0.200 by IMx CORE-M had hepatitis, and those with cut-of index below 0.150 did not have hepatitis. 2.500 is suitable for cut-off level at diagnosis of AH-B in Japan, although perfect differential diagnosis by IMx CORE-M alone was considerably impossible.
