Möbius syndrome is a rare congenital bilateral facial palsy. It has been reconstructed using several techniques including local muscle transfer and free muscle transplantation. We report a case of Mobius syndrome reconstructed by free muscle transplantation. A 19-year-old woman had a congenital bilateral facial palsy, poor tongue motion and normal masseter. The treatment procedure used was a free bilateral latissimus dorsi muscle transplant and we chose the masseter motor nerve of as the recipient nerve. The patient gained an early "smile" and was satisfied with this result.
