We describe a case of adenocarcinoma of the gallbladder that originated in and limited to the Rokitansky-Aschoff sinus (RAS) of adenomyomatosis. CT and MRI showed many intramural stone and cystic structures which represent the RAS in the thickened gallbladder wall, but did not reveal malignant lesion. Microscopically, atypical cells with papillary projections were found within some of the RAS which had stone. The increased risk of gallbladder cancer for adenomyomatosis with many intramural stones might have important clinical implications.
